COVID-19 Vaccination Scheduling


Welcome to the Atrius Health COVID-19 Vaccination Scheduling website, offered in conjunction with the Massachusetts Department of Public Health (DPH, mass.gov).

This site is restricted for use by individuals currently eligible to be vaccinated in accordance with the Massachusetts COVID-19 Vaccination Distribution and Prioritization Plan. Please check this site to confirm your eligibility before proceeding with scheduling.

As of April 19th, patients who live, work, or study in Massachusetts aged 16 and above are eligible for a COVID-19 vaccine.

Please note, this location can only vaccinate patients aged 18 and above at this time.

If you are scheduling immunization at this location you will receive the Moderna COVID-19 vaccine. This vaccination requires two doses 28 days apart and both doses must be administered at this same Harvard Vanguard Needham location.

Only the first dose can be scheduled through this website. The second dose will be scheduled in person on the day of your first appointment after receiving the first dose. You may receive an automated phone call or text reminder from Atrius Health 2 days prior to your scheduled appointment time.

Please bring an ID with your name on it if you have one.


The Commonwealth requires you to confirm you have reviewed the eligible phases outlined by their framework, and attest under penalty of perjury that you fall into a category currently eligible for scheduling. Please read and acknowledge the following:


I can be available for the second dose of the Moderna COVID-19 vaccine 28 days from the date of my first scheduled appointment. Appointments can be scheduled between 7:00 am and 5:00 pm, Monday through Saturday.

I confirm that:
  • I have not had a severe allergic reaction to a mRNA COVID-19 vaccine in the past (Pfizer BioNTech or Moderna)
  • I did not previously receive COVID-19 antibody therapy, including monoclonal antibody or convalescent plasma therapy, in the 90 days prior to my vaccination date.
  • I do not have history of recent (within the past 20 days) or current documented COVID-19 disease
  • I have not received/will not receive any other vaccination within 14 days of either dose of COVID-19 vaccination
  • I have not been exposed to someone confirmed to have COVID-19 within the prior 14 days
  • I understand that I may wish to have a discussion with my healthcare provider(s) before vaccination if I have a condition or take medicines that weaken my immune system, am currently pregnant, breastfeeding, or planning to become pregnant
  • I have read the EUA fact sheet for recipients of the Moderna COVID-19 vaccine (opens in new window)

A current email address is required for online scheduling. We will send an appointment confirmation including the date, time and location of your scheduled immunization to the email address you provide. We may also send occasional Atrius Health email communications such as our Healthy Matters e-newsletter and practice updates. You can opt out of these email communications when received.
