What is Proxy Access?

   Proxy access allows someone to view the medical records of a family member or person under their care, and communicate with the patient's physician's office on their behalf.
   If you have proxy access to an Atrius Health patient, you can use MyHealth Online to view portions of that patient's medical records and communicate with their physician's office.  All MyHealth Online communications are restricted to non-urgent matters.
   Proxy access is granted once an authorization form has been completed and processed by the site where the patient receives care.  Proxy access can be terminated by Atrius Health at any time.

   You can access the accounts you have been given proxy access to by logging into your own MyHealth account.

Types of Proxy Access We Offer


Pediatric Proxy

Pediatric proxy access for MyHealth Online allows a parent or guardian to securely communicate on behalf of an Atrius Health pediatric patient who is under the age of 13, and access components of their medical record. Access is obtained by completing the Pediatric Proxy Authorization Form, or the account may be created directly during an office visit for your child.  Access is automatically transitioned to Adolescent Proxy Access when your child turns 13 years old.

Pediatric Proxy Terms and Conditions

  • The proxy requestor must be the parent or legal guardian of the pediatric patient. If the requestor is not a parent or legal guardian, please call the number listed on the Atrius Health Site Location Information Sheet.
  • The proxy requestor must complete and sign the Pediatric Proxy Authorization Form, or have the account created in the office.
  • Each proxy requestor must have an active MyHealth Online account but, does not need to be an Atrius Health patient.
  • Each proxy requestor must submit one form per child.
  • Pediatric proxy access is automatically transitioned to Adolescent Proxy Access when your child turns 13 years old.
  • Proxy access can be terminated, online or by written request.

How do I obtain Pediatric Proxy access for MyHealth Online?

  • The parent or legal guardian must complete and sign the Pediatric Proxy Authorization Form; one for each child, or have the account created in the office.
  • Parent or legal guardian can drop off or mail the form to the health site where the patient receives care or to the appropriate MyHealth Office listed on the Atrius Health Site Location Information Sheet.
  • Upon receipt requests are processed within 3-5 business days. Once processed, an access code and instructions will be forwarded to the proxy at the email address provided on the proxy authorization form.

Adolescent Proxy Access

Adolescent proxy access for MyHealth Online allows the parent or legal guardian to view a limited portion of the adolescent's medical record, and communicate with the physician's office. This is to protect an adolescent's right to privacy. Parents will maintain proxy access if signed up prior to the patient turning 13 years old but the view of information will then be limited to the adolescent proxy access.  If the parent would like a new proxy access account after the child turns 13 years old then access may be obtained in the office or by filling out the Adolescent Proxy Authorization Form in the office.

Adolescent proxy access is terminated when the adolescent turns 18 years old or when the parent/legal guardian makes a written or online request to terminate access. An emancipated minor may request that proxy access be terminated.

Adolescent Proxy Terms and Conditions

  • Proxy access may be created during an office visit or the parent/legal guardian must complete the form.
  • Each parent/legal guardian requesting access must have an active MyHealth Online account but, does not need to be an Atrius Health patient.
  • Adolescent proxy access can be terminated only by the parent/legal guardian at anytime online or by written request, unless the adolescent has been determined to be an emancipated minor.

How to obtain Adolescent Proxy for MyHealth Online?

Proxy access may be created during an office visit or the parent/legal guardian must complete the Adolescent Proxy Authorization Form.

Adolescent Proxy access will enable:

  • Adolescents (full access):
    • View growth charts, immunizations, allergies and medications
    • Communicate with your physician's office on non-urgent issues
    • View lab results
    • Schedule and request appointments
    • View upcoming and past appointment details, cancel appointments
    • View School/Camp forms and letters
       
  • Parents/Legal Guardians (limited access):
    • View growth charts, immunizations and allergies
    • Communicate with the physician's office on non-urgent issues
    • View lab results
    • Schedule appointments (but not view upcoming or past appointments, or cancel appointments)

Adult Proxy

Adult proxy access for MyHealth Online allows you to securely communicate on behalf of an Atrius Health patient who is 18 years of age or older, and view components of their medical record. In order to obtain access, both the patient and proxy must complete and sign the Adult Proxy Authorization Form. The proxy's access is terminated when the patient makes a written or online request to terminate access, an expiration date specified by the patient is reached, or the patient revokes access from within their MyHealth Online account.

Adult Proxy Terms and Conditions

  • The proxy requestor and patient must both complete and sign the Adult Proxy Authorization Form. If the patient is unable to complete this form, please call the number listed on the Atrius Health Site Location Information Sheet.
  • Each proxy requestor must have an active MyHealth Online account but, does not need to be an Atrius Health patient.
  • Each proxy requestor must submit one form per patient.
  • Proxy access can be terminated by the patient at anytime online or by written request.

How do I obtain Adult Proxy access for MyHealth Online?

  • The proxy requestor and patient must both complete and sign the Adult Proxy Authorization Form. If the patient is unable to complete this form, please call the number listed on the Atrius Health Site Location Information Sheet.
  • The patient or proxy requestor can drop off or mail the form to the health site where the patient receives care or to the appropriate MyHealth Office listed on the Atrius Health Site Location Information Sheet.
  • Upon receipt, requests are processed within 3-5 business days upon receipt. Once processed the patient is called to verify access authorization and then access information is sent via U.S.Mail to the patient.

I have signed the authorization form for MyHealth Online Proxy Access, where do I send the completed form?

Please send the completed authorization form to the administrative office of the primary care site where the patient receives care. Alternatively, the forms can be physically taken to the appropriate site were the patient receives care.

I am not a patient of Atrius Health; can I obtain proxy access?

Yes. You will be required to complete either the Pediatric Proxy Authorization Form, Adolescent Proxy Authorization Form or the Adult Proxy Authorization Form and check off the box which states that you are not an Atrius Health patient. Once the appropriate form has been completed and approved, the form may be mailed or physically brought to the site were the patient receives care.

I have been given proxy access, but don't know how to view or activate the patient's information, please advise?

  1. Login to your MyHealth Online account with your MyHealth Online ID (username) and password
  2. Click on a "Proxy Tab," near the top of the page, with the name of the person you want to access. (You can customize name and colors by clicking the "Wrench" icon ).
