MyHealth Online: Patient Guidelines and Terms and Conditions for Use
By accessing or using MyHealth Online in any format, either via web browser, mobile device application, or other interface, you acknowledge and agree to comply with the guidelines stated below and all the terms and conditions of this Agreement.
You agree NOT TO SHARE your password with anyone. Sharing your password will enable others to have access to your personal confidential medical information. You are solely responsible for protecting your password.
You agree not to send any messages requiring urgent attention. Delays can
occur based upon volume, availability of staff and the complexity of your condition. If you need a response in less than two (2) business days please contact your clinician's office directly by telephone. Our regular business hours are Monday through Friday 8 a.m. to 5 p.m.
For all urgent medical matters, contact your clinician's office.
You agree that MyHealth Online is a communication service offered as a convenience to patients. You understand that it provides you with online access to portions of your electronic medical record and that you are solely responsible for any sharing of MyHealth Online content that you intentionally or unintentionally communicate to others.
You understand that messages will be reviewed by your healthcare team, including Medical Assistants, to determine who can best respond to your message. Your message will be routed to the appropriate individual and/or department for handling, and therefore, some employees, other than your healthcare provider may be involved in addressing your request. If you only want a particular clinician to receive the information you should not send such information through MyHealth Online. Remember, use of MyHealth Online may not be appropriate if there is sensitive information that you would only wish to discuss directly with your provider.
You understand that you may receive messages, test results and other communications delivered via MyHealth Online. If you receive this information through MyHealth Online, in general you will not receive separate written communication regarding this information from your health care team.
You understand that messages you send via MyHealth Online may be made part of your permanent medical record at the discretion of the clinician. Once it is made part of your record, it will be accessible to current and future staff members who are appropriately involved with your care.
You understand that not all of your medical record will appear on your MyHealth Online. Posting of information is strictly at the discretion of your individual clinician.
All communications between you and your health care team and access to your personal health information using MyHealth Online are carried over a secure, encrypted connection directly into a segregated section of the electronic medical record. While we use state-of-the-art security, no system can perfectly guard against risks of intentional intrusion or inadvertent disclosure of information. When using MyHealth Online, information will be transmitted over a medium that is beyond the control of its contractors. YOU HEREBY EXPRESSLY ASSUME THE SOLE RISK OF ANY UNAUTHORIZED DISCLOSURE OR INTENTIONAL INTRUSION, OR OF ANY DELAY, FAILURE, INTERRUPTION OR CORRUPTION OF DATA OR OTHER INFORMATION TRANSMITTED IN CONNECTION WITH THE USE OF THIS SERVICE. Once the information is received by us, your medical information will be treated as confidential and given the same protection that all other electronic medical records are given. While you may receive Internet e-mail messages notifying you of new messages in your MyHealth Online Inbox, these e-mails may contain confidential medical information.
When you initially enroll to use MyHealth Online, you will be provided a temporary access code. It is extremely important that you keep your MyHealth Online temporary access code, MyHealth Online ID (username) and password in a secure location and completely confidential. Anyone with access to your temporary access code, MyHealth Online ID (username) and password will be able to view your medical information and communicate with your health care team as if that person were you. It is your responsibility to prevent disclosure of your temporary access code and password and to change your password if you feel that your security has been compromised. You can change your password on the MyHealth Online web site at any time by choosing "Change password" under the preferences options.
Your login name and password are key to protecting the confidentiality of your information. You are responsible for keeping your password confidential. It is only known by you and can only be changed by you online. Your password is not known by anyone at our organization.
As we provide your health care, we are required to maintain documentation of your medical history, current conditions, treatment plan and all treatments given, including the results of all tests, procedures and therapies. Messages you send via MyHealth Online may be made part of your permanent medical record at the discretion of the clinician. Once it is made part of your record, it will be accessible to current and future staff members who are appropriately involved with your care.
We are deeply committed to protecting our patient's rights to privacy, and to safeguarding patient information. We have detailed policies and procedures regarding access to all records by our workforce. These policies conform to state and federal law and are designed to safeguard your privacy.
Our workforce is educated in confidentiality policies and the appropriate use of medical information. Our patients medical information is held in strictest confidence.
All electronic medical record content is subject to all state and federal statutes governing the security and confidentiality of medical records.
Discontinuing Use of MyHealth Online
You may discontinue your use of MyHealth Online at any time by sending a secure message to your PCP's office requesting this change.
You agree that you will not post or transmit any offensive material from or related to this site. If we determine that you have violated this agreement and/or abused the use of this service, we reserve the right, at our sole discretion, to discontinue your use of this service. You will be notified if your MyHealth Online service is discontinued.
Copyright and Trademarks
All content included on our public websites, and any MyHealth Online services, including, but not limited to, text, photographs, graphics, button icons, images, artwork, names, logos, trademarks, service marks and data (the "Content"), in any form including the compilation thereof, are protected by U.S. and international copyright law and conventions.
Site Access and Licenses
We grant a limited license to each user to make personal use only of the MyHealth Online website, mobile application, or any other interface, and any associated services in accordance with these terms and conditions of use. This license expressly excludes, without limitation, any reproduction, duplication, sale, resale or other commercial use of the website and the associated services, making any derivative of the website or the associated services, the collection and use of user email addresses or other user information, including, without limitation, health information or any data extraction or data mining whatsoever.

MyHealth Online contains links to websites operated by other parties. The links are provided for your convenience only. We do not control such websites and we are not responsible for the content and performance of these sites. The inclusion of links to other websites does not imply any endorsement of the material on the websites or any association with their operators. We do not operate, control or endorse any information, products or services provided by third parties through the Internet. Use of other sites is strictly at your own risk including, but not limited to, any risks associated with destructive viruses. You are responsible for viewing and abiding by the terms and conditions of use and the privacy statements of the other websites.
MyHealth Online and all other sites and services hosted by our organization, and the content contained herein and therein are provided on an As Is basis. WE MAKE NO REPRESENTATIONS OR WARRANTIES OF ANY KIND, EXPRESS OR IMPLIED AS TO THE OPERATION OF THE SITES, OR THE CONTENT, PRODUCTS AND/OR SERVICES INCLUDED THEREIN. You expressly agree that exclusive jurisdiction for any dispute with our organization, its affiliates, employees, contractors, officers and directors, resides in the courts and you further agree and expressly consent to the exercise of personal jurisdiction in the courts in connection with any claim involving our organization, its affiliates, employees, subsidiaries, contractors, officers and directors. Any use of our public services is unauthorized in any jurisdiction that does not give effect to the terms and conditions set forth herein.
