Proxy Access Guidelines

What is Proxy Access?

Proxy access allows parents and legal guardians to view the medical records of a family member or person under their care, and communicate with the patient's physician's office on their behalf. If you have proxy access to an Atrius Health patient, you can use MyHealth Online to view portions of that patient's medical records and communicate with their physician's office. Parents/guardians will receive email notifications of new information in their proxy accounts (i.e., a new test result).


Obtaining proxy access to a patient 17 years old or under can be done in person, at the child's Primary Care office (no form required), or by mailing in a proxy request form. Proxy access to an adult patient needs to have an Adult Proxy Access Form completed. See details below on the types of proxy access we offer.


You can access the patients you have been given proxy access to by logging into your own MyHealth Online account. You must have, and sign into, your own MyHealth Online account to access your proxies. You can create a MyHealth Online account even if you are not an Atrius Health patient yourself.

Types of Proxy Access We Offer

  • Pediatric Proxy Access - for patients age 12 and under.
    Parents or legal guardians are granted full access to the child's account.
    When the child turns 13, access will transition to the limited Adolescent Proxy Access. See details below.
  • Disabled Adolescent Access - for patients 13 to 17 years old but are not intellectually or physically capable of managing their health information. Parents or legal guardians are granted full access to the adolescent's account. Access ends when the patient turns 18. See details below.
  • Adolescent Proxy Access - for patients 13 to 17 years old.
    Parents or legal guardians are granted limited access to an adolescent/teenager's account. Access ends when the patient turns 18. See details below.
  • Adult Proxy Access - for patients age 18 or older.
    Adults are granted full access to another adult patient's account. See details below.

Pediatric Proxy (age 12 and under)

Pediatric proxy access for MyHealth Online allows a parent or guardian to securely communicate on behalf of an Atrius Health pediatric patient age 12 or under, and access components of their medical record. Pediatric Proxy Access will automatically transition to the more limited Adolescent Proxy Access when the child turns 13 years old.


How to obtain Pediatric Proxy Access to a patient age 12 or under:

  • Parent or legal guardians can obtain Pediatric Proxy Access either
    - in person at the child's Primary Care office (no form required), or
    - by completing the Pediatric Proxy Access Authorization Form and mailing it to the administrative office of the child's Primary Care location.
  • Disabled Adolescent Access must complete the Pediatric and Disabled Teen Proxy Authorization Form, and the child's provider must also sign the form.
  • Proxy request forms are processed within 3-5 business days. Once processed, instructions will be sent via email to the requester.

Pediatric Proxy Access Terms and Conditions

  • The proxy requester must be the parent or legal guardian of the pediatric patient. If the requester is not a parent or legal guardian, please call the child's Primary Care office.
  • Each proxy requester must have their own active MyHealth Online account. They do not need to be an Atrius Health patient.
  • Pediatric Proxy Access is automatically transitioned to the limited Adolescent Proxy Access when the child turns 13 years old.
  • Proxy access can be terminated by a proxy via MyHealth Online or written request.
    Atrius Health can terminate proxy access at any time for any reason.

Adolescent Proxy Access (13-17 years old)

Adolescent proxy access for MyHealth Online allows a parent or legal guardian to view a limited portion of the adolescent's medical record, and communicate with the physician's office of the patient's behalf. The limitations (outlined below) are is to protect adolescent's right to privacy, pursuant to Massachusetts law and Atrius Health policies.


Parents with existing Pediatric Proxy Access to a child are automatically transitioned to the more limited Adolescent Proxy Access when the child turns 13. Adolescent Proxy Access ends when the patient turns 18 years old.


How to obtain Adolescent Proxy Access to patient 13-17 years old:

  • Parent or legal guardians can obtain Adolescent Proxy Access either
    - in person at the adolescent's Primary Care office (no form required), or
    - by completing the Adolescent Proxy Access Authorization Form and mailing it to the administrative office of the adolescent's Primary Care office.
  • Proxy request forms are processed within 3-5 business days. Once processed, instructions will be sent via email to the requester.

Adolescent Proxy Access Terms and Conditions

  • The proxy requester must be the parent or legal guardian of the adolescent patient.
  • Each parent/legal guardian requesting access must have their own active MyHealth Online account, but does not need to be an Atrius Health patient.
  • Adolescent Proxy Access can be terminated by the parent/legal guardian at any time via MyHealth Online or by written request, unless the adolescent has been determined to be an emancipated minor.
    Atrius Health can terminate proxy access at any time for any reason.

Adolescent Proxy Access feature limitations:   (pursuant to Massachusetts law and Atrius Health policies)

  • Communicate with the teen's physician's offices about non-urgent issues
  • View growth charts, immunizations, allergies, and health trends
  • View lab results
  • Schedule appointments
  • Preventive care appointment reminders
  • View letters (including school/camp forms)
  • View medications
  • View future or past appointments
  • View current health issues
  • Cancel or confirm future appointments
  • Pre-visit updates
  • Add/remove allergies

Adolescents themselves can create their own MyHealth Online accounts, during an office visit, and log in with near-full access.

Adult Proxy (age 18 and over)

Adult Proxy Access allows you to securely communicate on behalf of an Atrius Health patient who is age 18 or older, and view components of their medical record. The proxy's access is terminated when an expiration date specified by the patient is reached, or the patient revokes access from their MyHealth Online account.


How do I obtain Adult Proxy Access for MyHealth Online?

  • Both the patient and proxy must complete and sign the Adult Proxy Authorization Form. The patient or proxy requester can drop off or mail the form to the administrative office of the patient's Primary Care location.
  • Requests are processed within 3-5 business days of receipt. Once processed the patient is called to verify access authorization and then access information is sent via U.S. Mail to the patient.

Adult Proxy Access Terms and Conditions

  • The proxy requester and patient must both complete and sign the Adult Proxy Authorization Form.
  • Each proxy requester must have an active MyHealth Online account but, does not need to be an Atrius Health patient.
  • Each proxy requester can submit one patient per form.
  • Proxy access can be terminated by the patient or proxy via MyHealth Online or written request.
    Atrius Health can terminate proxy access at any time for any reason.


Where to send the completed proxy form

Please send the completed authorization form to the administrative office of the primary care site where the patient receives care - see the last page of the request forms for addresses. The forms can also be physically taken to the office were the patient receives primary care.

I am not a patient of Atrius Health; can I obtain proxy access?

Yes. Complete the appropriate proxy access request form and check off the box which states that you are not an Atrius Health patient. The form may be mailed or physically brought to the site were the patient receives primary care. You will receive instructions on how to set your own MyHealth account; always sign into your own account to access your proxies.
Non-patients cannot use the MyHealth Online application system to apply for an account.

How to access a proxy account

  • Login to your own MyHealth Online account with your own MyHealth Online ID (username) and password.
  • Click on a "Proxy Tab" at the right of the page, with the name of the person you want to access.
