Proxy Access Guidelines

What is Proxy Access?

Proxy access allows parents and legal guardians to view the medical records of a family member or person under their care, and communicate with the patient's physician's office on their behalf. If you have proxy access to an Atrius Health patient, you can use MyHealth Online to view portions of that patient's medical records and communicate with their physician's office.


Proxy access is granted once an authorization form has been completed and processed by the site where the patient receives care.  Proxy access can be terminated by Atrius Health at any time.


You can access the accounts you have been given proxy access to by logging into your own MyHealth account (even if you are not an Atrius Health patient).

Types of Proxy Access We Offer

  • Pediatric Proxy Access - for patients age 12 and under. (view form)
    Parents may enroll to have full access to a child's account.
    When the child turns 13, access will transition to "Adolescent Proxy Access."
  • Disabled Adolescent Access - for patients who are 13 to 17 years old but are not intellectually
    or physically capable of managing their health information. (view form)
  • Adolescent Proxy Access - for patients who are 13 to 17 years old. (view form)
    Parent and teen must enroll together and in person at the pediatrician's office.
    The parent will have a limited view of the teen patient's records.
    If the teen choses to activate their own account, they have full MyHealth access to their own records.
  • Adult Proxy Access - for patients who are 18 years of age or older. (view form)

Pediatric Proxy (age 12 and under)

Pediatric proxy access for MyHealth Online allows a parent or guardian to securely communicate on behalf of an Atrius Health pediatric patient who is under the age of 13, and access components of their medical record. Access is obtained by completing the Pediatric Proxy Authorization Form, or the account may be created directly during an office visit for your child.  Access is automatically transitioned to Adolescent Proxy Access when your child turns 13 years old.


Pediatric Proxy Terms and Conditions

  • The proxy requester must be the parent or legal guardian of the pediatric patient. If the requester is not a parent or legal guardian, please call the number listed on the Atrius Health Site Location Information Sheet.
  • The proxy requester must complete and sign the Pediatric Proxy Authorization Form, one for each child, or have the account created in the office.
  • Each proxy requester must have an active MyHealth Online account but, does not need to be an Atrius Health patient.
  • Each proxy requester must submit one form per child.
  • Pediatric proxy access is automatically transitioned to Adolescent Proxy Access when your child turns 13 years old.
  • Proxy access can be terminated, online or by written request.
  • Disabled Adolescent Access use the Pediatric Proxy form. If deemed disabled by his/her provider, the provider must sign the form.

How do I obtain Pediatric Proxy access for MyHealth Online?

  • Parent or legal guardian can drop off or mail the form to the health site where the patient receives care or to the appropriate MyHealth Office listed on the Atrius Health Site Location Information Sheet.
  • Upon receipt requests are processed within 3-5 business days. Once processed, instructions will be sent to the proxy at the email address provided on the proxy authorization form.

Adolescent Proxy Access (13-17 years old)

Adolescent proxy access for MyHealth Online allows the parent or legal guardian to view a limited portion of the adolescent's medical record, and communicate with the physician's office. This is to protect an adolescent's right to privacy. Parents will maintain existing proxy access to patients turning 13 years old, but the view of information will be limited to adolescent proxy access. Proxy access for a new adolescent patient (not previously a pediatric proxy) is obtained by completing the Adolescent Proxy Authorization Form with both adolescent and adult in person in the office.


Adolescent proxy access ends when the adolescent turns 18 years old or when the parent/legal guardian makes a written or online request to terminate access. An emancipated minor may request that proxy access be terminated.


Adolescent Proxy Terms and Conditions

  • Proxy access may be created during an office visit or the parent/legal guardian must complete the form.
  • Each parent/legal guardian requesting access must have their own active MyHealth Online account, but does not need to be an Atrius Health patient.
  • Adolescent proxy access can be terminated only by the parent/legal guardian at anytime online or by written request, unless the adolescent has been determined to be an emancipated minor.

How to obtain Adolescent Proxy for MyHealth Online

Completed forms may be submitted with both the adolescent and adult in the office.


Adolescent Proxy access will enable parents/legal guardians limited access, including:

  • View growth charts, immunizations and allergies
  • Communicate with the physician's office on non-urgent issues
  • View test results
  • Schedule appointments (but not view upcoming or past appointments, or cancel appointments)

Adolescents themselves can create their own MyHealth Online accounts and log in with full access.

Adult Proxy (18 years old and over)

Adult proxy access for MyHealth Online allows you to securely communicate on behalf of an Atrius Health patient who is 18 years of age or older, and view components of their medical record. In order to obtain access, both the patient and proxy must complete and sign the Adult Proxy Authorization Form. The proxy's access is terminated when the patient makes a written or online request to terminate access, an expiration date specified by the patient is reached, or the patient revokes access from within their MyHealth Online account.


Adult Proxy Terms and Conditions

  • The proxy requester and patient must both complete and sign the Adult Proxy Authorization Form. If the patient is unable to complete this form, please call the number listed on the Atrius Health Site Location Information Sheet.
  • Each proxy requester must have an active MyHealth Online account but, does not need to be an Atrius Health patient.
  • Each proxy requester must submit one form per patient.
  • Proxy access can be terminated by the patient at anytime online or by written request.

How do I obtain Adult Proxy access for MyHealth Online?

  • The patient or proxy requester can drop off or mail the form to the health site where the patient receives care or to the appropriate MyHealth Office listed on the Atrius Health Site Location Information Sheet.
  • Requests are processed within 3-5 business days of receipt. Once processed the patient is called to verify access authorization and then access information is sent via U.S.Mail to the patient.

Where to send the completed proxy form:

Please send the completed authorization form to the administrative office of the primary care site where the patient receives care. Alternatively, the forms can be physically taken to the appropriate site were the patient receives care.

I am not a patient of Atrius Health; can I obtain proxy access?

Yes. Complete the appropriate proxy access form and check off the box which states that you are not an Atrius Health patient. The form may be mailed or physically brought to the site were the patient receives care. You will receive instructions on how to set your own MyHealth account; always sign into your own account to access your proxies.
You cannot use the MyHealth Online application to apply for an account.

How to access a proxy account:

  1. Login to your MyHealth Online account with your own MyHealth Online ID (username) and password.
  2. Click on a "Proxy Tab" at the right of the page, with the name of the person you want to access.
    Tip: you can customize names, colors and photos by clicking the "Wrench" icon .
